Your Opportunity
We are seeking a Home Health C.NA whose primary responsibility is to performs routine duties in assisting Residents who reside in Fleet Landing.

Key Responsibilities:
Assist with facilitating Resident and family communication with and between medical staff.
Immediately report all changes in Resident’s condition to the on-call RN, or supervisor.
Perform chore services or companionship per care-plan created by agency and Resident.
Review resident plan of care weekly to ensure appropriate and requested services are being provided. If resident or family member requests an addition to the care plan, supervisor must be notified; Supervisor will reach out to client or family member to ensure needs are met. Ensures written documentation reflects that the plan of care is being followed.
Observes areas of skin breakdowns and report concerns to the on call nurse or supervisor.
Informs supervisor or scheduler of supply/equipment needs and reports defective equipment.
Ensures fairness in treatment; listens to residents; gives choices and responds to individual preferences and requests.
Qualifications:
Must have a current CNA in the state of Florida
Minimum Education Required:
High school diploma or equivalent. Possess a valid current CPR certification.
Experience:
Minimum of 6 months, preferred.
